Informed Consent for Hwarang Youth Foundation
Consent and Release:
I agree to participate in the activities and events organized by Hwarang Youth Foundation.
I understand and acknowledge the following:

Voluntary Participation: My participation in any event or activity organized by Hwarang Youth Foundation is entirely voluntary. I may withdraw from participation at any time without any consequences.

Assumption of Risk: I understand that participation in activities and events may involve risks, including but not limited to physical injury, illness, or loss of personal property. I assume all risks associated with my participation.

Medical Treatment: In the event of an emergency, I consent to the provision of first aid and, if necessary, medical treatment. I understand that the Hwarang Youth Foundation will make reasonable efforts to contact my emergency contact.

Photography and Media Release: I grant Hwarang Youth Foundation permission to photograph, film, or otherwise record my participation in events and activities. I consent to the use of these recordings for promotional, educational, and informational purposes.

Confidentiality: Hwarang Youth Foundation respects the confidentiality of its participants. Any personal information provided will be kept confidential and used solely for the purposes of communication and participation in organization activities.
